What Research Says About ACL Recovery

Modern ACL rehabilitation is criteria-based: you progress when your knee demonstrates it's ready, not just when the calendar says so. The research behind that approach is clear:

  • Most athletes need 6–9 months before safely returning to competitive sport. Returning earlier carries measurably higher risk: one landmark study found that each month return to sport was delayed (up to 9 months post-op) reduced re-injury risk by about half (Grindem et al., British Journal of Sports Medicine, 2016).

  • Young athletes who return to cutting and pivoting sports face a roughly 1-in-4 risk of a second ACL injury — to either knee — which is exactly why testing before return matters (Wiggins et al., American Journal of Sports Medicine, 2016).

  • Athletes who pass objective return-to-sport criteria — strength within ~10% of the uninjured side, symmetrical hop testing, sport-specific movement quality — return at significantly lower re-injury risk than those cleared on time alone.

That's the standard we rehab to: objective strength testing at each phase, movement quality you can see and measure, and a graded return through practice before competition.

How We Progress With You: Benchmarks, Not Dates

At Premier Performance, ACL rehab isn't run on a calendar. You advance when your knee proves it's ready — measured, not assumed. Two athletes with the same surgery date can be months apart in their progression, and that's the point: the plan fits your knee, not an average.

Early phase: full knee extension, swelling controlled, quad firing again. Nobody progresses past this until it's done — it sets the ceiling for everything after.
Strength phase: progressive loading until objective strength testing clears you for impact — running is earned, not scheduled.
Power & agility phase: jumping, cutting, and change-of-direction progressions, each gated by testing against your uninjured side, or previous history.
Return-to-sport phase: a formal testing battery depending on the sport — strength symmetry, hop testing, movement quality — before clearance, then a graded return through practice before competition.

Most athletes who start early rehab with us land in the 6-9 month range — biology takes the time it takes — but the date is the result of passing benchmarks, never the reason for clearance.

ACL Rehab: Common Questions

Can a torn ACL be rehabbed without surgery?

Sometimes, yes. A subset of patients — often called "copers" — return to full activity with structured rehab alone, depending on the injury, the sport, and the demands you're returning to. We help you test whether non-operative care is realistic before anyone commits to surgery.

1

How long until I can return to sport after ACL surgery?

Most athletes who start early rehab with us are in the 6-9 month range — but at our clinic that's an outcome, not a deadline. You're cleared when you pass objective testing against your uninjured side, not when a date arrives. Returning before you're demonstrably ready is the single biggest re-tear risk. Patients who start later in the rehab process generally have longer time lines.

2

When should physical therapy start after surgery?

Usually within the first couple days post operative. Early goals — full extension, swelling control, quad activation — set up everything that follows. Some patients benefit from pre-op therapy as well.

3

What should good ACL rehab include?

Progressive strength work, objective testing at each phase, and a graded return through practice before competition — not just exercises until pain stops.

Reviews

“I had surgery in December on both my ACL and Meniscus and I was recommended by a friend of mine to go to PPPT. Working with Greg has been fantastic as I see myself progressing weekly and I’m starting to feel more confident in my knee after every session. At PPPT they make sure to push you (within comfort) and get the most out of every session. Highly recommend to anyone looking for optimal rehab!”

“Greg has been extremely awesome to work with.  After spending time at a different facility after my torn ACL and meniscus.  I felt like I was still far behind in my rehab.  That changed very quickly once I switched to Premiere Performance.  The biggest difference with Greg is the way he pushes me to improve.  He has knowledge and expertise to assist anyone on their way to recovery.  I would highly recommend him.”
